Documents required at the time of admission

A) Candidates Graduated from Mumbai University should submit 2 SETS OF ATTESTED COPIES of following documents along with ORIGINAL documents.
 
  1. Seat Allotment Letter (Computer Print Out)
  2. CET Final Score Card
  3. Certificate of Indian Nationality & Domicile Certificate
  4. S.S.C. & H.S.C. Mark sheets
  5. Graduation Final Year Mark sheet & Degree Certificate
  6. Date of Birth proof ( from academic institution/college )
  7. Gap Certificate(if graduated before March / April 2011)
  8. Transference Certificate (T.C. form should be collected from office and T.C. should be submit within 15 days from admission date.
  9. Caste Certificate & Caste Validity Certificate for Reserved Category Candidates or Undertaking in Proforma ‘H’
  10. Creamy Layer Certificate valid upto 31st March, 2012 (if candidate belongs to VJ/DT(A)/NT(B)/NT(C)/NT(D)/OBC/SBC category
  11. Undertaking in Proforma ‘I’ in original on Rs. 100/- non-judicial Stamp Paper.
  12. Free ship Form / Scholarship Form (if seat allotted under Reserved Category) along with required documents. (See Free ship/Scholarship Notice for details)
  13. P.G. Registration form, Admission Form & Undertaking (to be supplied by office)
  14. Passport size Photograph (2 copies)
  15. Fees by Demand Draft only in favour of “ Chetana’s R.K. Inst. Of Mgmt. & Res."
B) Candidates Graduated from other than Mumbai University and All India should submit 5 SETS OF ATTESTED COPIES of following documents along with ORIGINAL documents
 
  1. Seat Allotment order / (Computer printout of allotment by DTE).
  2. CET final Score Card.
  3. CAT Score Card along with Converted Score attested by DTE (if applicable)
  4. Certificate of Indian Nationality
  5. S.S.C. & H.S.C. Mark sheets
  6. Graduation Mark sheet [All Semester wise /Year wise ] & Degree Certificate
  7. Date of Birth proof ( from academic institution/college )
  8. Gap Certificate (if graduated before March / April 2011)
  9. Caste Certificate & Caste Validity Certificate for Reserved Category Candidates
  10. Creamy Layer Certificate valid upto 31st March, 2012 (if candidate belongs to VJ/DT(A)/NT(B)/NT(C)/NT(D)/OBC/SBC category.
  11. Undertaking in Proforma ‘I’ in original on Rs. 100/- non-judicial Stamp Paper.
  12. Migration Certificate (should submit within 15 days from admission date)
  13. Free ship Form / Scholarship Form (if seat allotted under Reserved Category) along with required documents. (See Free ship/Scholarship Notice for details)
  14. P.G. Registration form, Admission Form ,Provisional Eligibility Certificate Form, Enrolment Form, Identity Card form & Undertaking to be filled in (forms will be supplied by office)
  15. Passport size Photograph (3 copies)

C. RESERVED CATEGORY CANDIDATES :

Should submit in addition to above documents, the following attested copies of documents at the time of admission to claim Freeship / Scholarship as applicable failing which they will have to pay full fees

  1. FREE SHIP APPLICATION FORM duly filled in and signed by candidate & parent and attested.
  2. Income Certificate (Form 16) of Financial Year 2010-11 of Parents
  3. Proof of Freeship amount received during previous year, if any
  4. Graduation Final Year Mark sheet
  5. Caste certificate & Caste Validity Certificate
  6. Creamy Layer Certificate valid upto 31st March, 2012 (if candidate belongs to VJ/ DT(A)/ NT(B)/ NT(C)/ NT(D)/ OBC/ SBC category
  7. District Change Certificate (if candidates graduated from other than Mumbai District).
  8. Copy of Ration Card
FOR SCHOLARSHIP :
  1. SCHOLARSHIP APPLICATION FORM duly filled in signed by candidate & parent .
  2. Income Certificate of Parents issued by Tahasildar and Collector for the Financial Year 2010-11
  3. Proof of Scholarship amount received during previous year, if any
  4. Graduation Final Year Mark sheet
  5. Caste certificate & Caste Validity Certificate
  6. Creamy Layer Certificate valid upto 31st March 2012 (if candidate belongs to VJ/ DT(A)/ NT(B)/ NT(C)/ NT(D)/ OBC/ SBC category

  6. Attested copy of Fee Receipt.
